A dedicated transport service for palliative and end of life care patients. It will provide a dedicated resource supported by staff with additional clinical and support skills in order to operate a flexible service that can respond to short notice requests for transport of palliative care patients. In addition to supporting the discharge of patients at the end of life, the service will also serve patients with complex needs e.g. those needing urgent assessment for spinal cord compression, management of pleural effusions and drainage of ascites.
The service will be expected to work in ways which are sensitive to the social, cultural and spiritual requirements of each patient and their carers. This will include access to translation and interpreting services.
